SUBMIT A REPORT

A grievence refers to a complaint or concern from a student, staff, or faculty member at NCCC.  If a student, staff, or faculty member believes that they have been negatively impacted or a policy/procedure has been violated, the individual may file a report. A WRITTEN REPORT should be completed whenever the following occurs:

Use our reporting form to document an incident of alleged academic dishonesty including documentation to support the violation of the Academic Integrity Policy (these reports are submitted by faculty). Full policy documents are located in the appendix of the Student Code of Conduct.

Reports of Classroom Misconduct should also be documented using our reporting process and coded as Student Behavior Report.

Use our reporting form to disclose COVID-19 exposure or a positive COVID-19 test result. Students may self-disclosure using this form or employees may document on a student's behalf.
Use our reporting form to disclose injury or illness onset experienced while on campus or during a college sponsored activity. Students may self-disclosure using this form or employees may document on a student's behalf.
Use our reporting form to disclose pregnancy/lactation status or parenting a child under 12 months of age. Disclosures enable a student to access Title IX accommodations for pregnant students or new parenting students. Self-referrals are encouraged. Faculty and Staff who learn of a disclosure are required to document their knowledge.

Use our reporting form to document when you have knowledge of a student whose needs rises to the level of concern that outreach and support may be needed to help a student stay safe, get emotional support, or be referred to a community agency for counseling. Student self-referrals also welcome.

Reports of concerning behavior or students suspected of being a threat to themselves or others should be reported immediate to the Student Life Office by calling 518-891-2915, ext. 1205 or the Administrator-on-Call at 518-354-0023 after hours.

The Behavioral Intervention Teams on each campus will coordinate care and outreach to support students struggling with addiction, mental health, suicide ideation and other concerns. 

Use our reporting form to document an incident of alleged behavioral misconduct which occurred on campus, online, in campus housing at the residence halls, in a student clinical settings, or at college sponsored events on or off campus.

Examples of such misconduct include but are not limited to harassment, assault, bias, violence, disruptive behavior, theft, or vehicle/property damage.

Students may self-report using this form. Employees must document the student behavior concerns for appropriate follow up.

Individuals are encouraged to file a report of any violence that occurs whether on or off campus. Those reporting acts of sexual or interpersonal violence may report anonymously but are encouraged to share as much information as possible in order for the College to thoroughly investigate claims and provide the best possible support. Once reported. the Title IX Coordinator will investigate if the case falls within our Title IX process of Student Code of Conduct process. More information available at nccc.edu/title-ix

Students are encouraged to self-disclose. Employees are deemed "responsible authorities" and are required to notify the Title IX Coordinator if they receive a disclosure of sexual misconduct.

If a student wishes to file a grievance, use our reporting form (below) to document student complaints. Complaints are routed to our Associate Vice President of Student Affairs for follow up. Students are welcome to self-report using this form as well.

North Country Community College is a member of the National Council for State Authorization Reciprocity Agreements (NC-SARA) and follows the complaint resolution policies and procedures outlined within the SARA Policy Manual and summarized here. Consumer protection complaints resulting from distance education courses, activities, and operations may be submitted to the Vice President for Academic Affairs for investigation and resolution. If a student is dissatisfied with the campus-based resolution, a concern may be submitted to System Administration of the State University of New York as outlined here. And, if still not satisfied, a complaint may be submitted, within two years of the incidentabout which the complaint is made, to the New York State Education Department as outlined here. Please note, this does not include complaints related to grades or student conduct violations, both of which are to be fully addressed via campus processes, not through the NC-SARA complaint resolution procedures.

Students with disabilities at NCCC have the right to fair and equal access in their educational setting. A grievance is a grounds for complaint where a student has an opportunity to state that something unfair or unjust has happened to them based on their disability. The grievance procedure is to deliver swift and impartial conclusions of complaints claiming any action prohibited by Section 504 of the Rehabilitation Act of 1973 (29 U.S.C. 794). If at any time a student feels that they have been discriminated against based on their disability, they have the right to file a grievance. A student can make this grievance either formally or informally.

 

STEPS TO FILING A REPORT:

1. Background Information is the person writing the report.
2. Involved Parties may include the person writing the report plus others (if applicable). Click "Add Another" to add more names.
3. Under Information, select Report Type to allow the form to dynamically change based on your selection. Then follow the prompts.
4. Attach any relevant materials, if appropriate, under Supporting Documentation.
 
Do NOT use this site to report events which present an immediate threat to safety or security. If this is an emergency, please contact Emergency Response by dialing 9-1-1. Urgent campus concerns may be reported by calling the Administrator-on-Call at 518-354-0023.
